What Is Preservé by Motiva®?
Preservé by Motiva® is an advanced breast implant surgery technique that uses state-of-the-art technology called the “Motiva® Channel Separator” to separate the tissue without the need for deep cuts. The “Motiva® Inflatable Balloon” is then inserted into this separation, creating a natural space big enough for your breast implant. This delicate, minimally invasive technique helps protect your natural breast tissue and its functions, such as nipple sensation.
Dr. Careaga performs Preservé by Motiva® using Ergonomix® implants only, placed above the muscle, with an implant volume limited to 320 cc. Round Plus implants and alternative placement options are not part of his Preservé technique.
The procedure uses a 2.5 cm incision and balloon dissection to create the implant pocket while preserving the circum-mammary ligament. The consultation determines whether this approach fits your anatomy and goals.

How Is Preservé Performed?
Dr. Careaga makes a 2.5 cm incision at the breast crease. Tissue separation and balloon dissection create the implant pocket above the muscle while preserving the circum-mammary ligament. He then places an Ergonomix® implant with a volume no greater than 320 cc and evaluates implant position, breast shape and symmetry before closing the incision.
Preparing for Preservé
Prior to surgery, Dr. Careaga will go over any pre-operative instructions with you. This may include:
- Medical testing
- Adjusting or stopping certain medications that increase bleeding
- Stopping smoking and substance habits to promote proper healing
- Arranging for transportation for the drive home and during early recovery
- Preparing a comfortable recovery area with loose clothing, prescribed medications, etc.
Follow any fasting, bathing, and arrival instructions from the team. You should also avoid wearing makeup, jewelry, or skin care products, as well as other external stimuli.
Carefully following your pre-operative instructions can help support a safer procedure and smoother recovery.
What to Expect During Preservé
On the day of your procedure, the Careaga Plastic Surgery team will review your surgical plan, selected implants, and pre-operative instructions before treatment begins. Once the implant has been placed, Dr. Careaga will evaluate its shape, position, and proportion before closing the incision.
The exact length of your procedure and type of anesthesia or sedation used will depend on your anatomy, health history, and personalized surgical plan.
After surgery, you will be monitored until you’re cleared to return home. Dr. Careaga will then provide detailed instructions for incision care, activity restrictions, medications, support garments, and follow-up appointments.
Preservé Recovery & Aftercare
Given its minimally invasive and smoother nature, the recovery process following Preservé is designed to be more comfortable and less disruptive than traditional breast augmentation.
While the approach minimizes trauma to the breast tissues, patients should still experience some degree of soreness, tightness, swelling, and bruising during the first several days. Dr. Careaga may also recommend that you wear a support garment, such as a surgical bra, to help protect your results and minimize swelling.
Light walking is generally encouraged to support circulation. However, any form of strenuous exercise or heavy lifting, including lifting your child, should be avoided until cleared by Dr. Careaga. Dr. Careaga reports a typical 48-hour return after Preservé. This is not a guarantee of full recovery or clearance for strenuous activity. Recovery varies, and you should follow his personalized activity restrictions and aftercare instructions.
Swelling will gradually improve within the first two weeks as the implant settles and breast tissues adjust. During this time, keep your incisions clean and attend all follow-up visits. Dr. Careaga will monitor your healing and provide personalized guidance on sleeping positions, exercise, and scar care.
What Results Can I Expect From Preservé?
Ultimately, Preservé creates fuller, more shapely breasts while maintaining a soft, natural-looking contour. Since this procedure moves the breast tissues around rather than cutting them, the implant involves more of the patient’s natural surrounding tissue.
Your results will be personalized according to your body proportions, existing tissue, and implant selection. Depending on your plan, this may mean improved breast size, projection, and symmetry in the upper pole. While some improvement may be visible shortly after surgery, swelling may hide your final results. As swelling subsides, your tissues will soften and settle into a more natural position.
Your final results may appear within the next several months. While results are long-lasting, factors such as aging, pregnancy, breastfeeding, weight changes, and hormonal fluctuations may affect your breasts over time. Maintaining a stable weight and healthy lifestyle can help protect your results.
